- cholera
- polio (given as part of the combined TD/IVP vaccination)

You MUST fill out this questionnaire at least 3 months before travel as we do not routinely stock travel vaccinations and they must be ordered into the practice.
If you are travelling in less than 3 months then please visit a travel health clinic for vaccination. This is a service offered by many local pharmacies.
