Please ask the reception whether you live in our Catchment area Chingford E4. If you live within our catchment area, you are welcomed to register. Eligibility can be confirmed from your address. We will not discriminate you on the grounds of race, religion, gender, class, age, medical condition, disability or sexual orientation.

How to register:

You can register online. To register please bring the following when prompted:

Photographic ID such as your passport, and one proof of address. It can be a bank statement, utility bill (gas, electricity, water), council bill or landline telephone bill. Unfortunately we do not accept mobile phone bills. If you were not registered in the UK you can bring EU card or home office papers. You can register online using the form below.

Our policy is that all patients registering with the practice might need to attend a routine medical check up with the practice and if taking medicine or undergoing any treatment might need to see the doctor as well. These checks should be done within three months of registering the practice. You may not be registered at the practice if you do not attend this health check.

We expect all patients undergoing treatment to have regular checks thereafter.

A new patient Health check for 0-5 years old: This form cannot be submitted electronically online. Please download the form and complete for all under 5's. kindly remember to bring your child's Red book and details of immunisations when attending the surgery.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
